The Traveling Child Returns with A Fever: Rats! Pediatric Hemorrhagic Fever Renal Syndrome Due to Hantavirus

A 6-year-old previously healthy female from the United States presented with 5 days of high fever to 103F, abdominal pain, and vomiting. Exposure history included recent travel Russia, Turkey, Yosemite, San Diego Zoo, as well multiple mosquito bites. On admission, she was found have thrombocytopenia, hematuria, nephrotic range proteinuria subsequently developed acute kidney injury. Her grandfather hospitalized shortly after her family’s departure Russia diagnosed hantavirus hemorrhagic renal syndrome (HFRS). While extremely rare in States, clinical picture concerning for HFRS. We also considered other infections thrombocytopenia such rickettsia, leptospirosis, dengue, hemolytic uremic syndrome, causes glomerulonephritis including lupus nephritis, post-streptococcus glomerulonephritis. After initial complications injury requiring support intravenous fluids, patient made an uneventful recovery. Serology tests later resulted positive Hantavirus. This case describes a diagnosis stresses importance HFRS should be on differential pediatric fever, or exposure.
